- At least two pages of the IRS Form 1040 that was used for the Financial Information part of the application and are presently being filed by the applicant.
If the test you are submitting is the ACT or SAT and your high school transcript does not include the results, you must independently upload the results of those tests to submit them.
Additionally, as part of the application process, you must submit one online recommendation by 2022 at 3 PM CT on your behalf.
